Union membership makes our collective voice stronger and more democratic. Consistent majority support and participation enabled us to negotiate new rights and improvements in our contracts, enable us to enforce these rights, and build a stronger union and a stronger school. I accept membership in the UAW and its designated Local, which represents me with respect to my working conditions at CalArts. I agree to pay a one-time $10 membership initiation fee and membership dues, currently 1.44% of my gross pay. My membership gives me voting rights in both the Local and the International.

Authorization for Deduction of Dues
I hereby authorize and direct the California Institute of the Arts (CalArts) to deduct from my earnings and to transmit to the UAW and its designated Local Union, dues and initiation fee in the amount established in accordance with the UAW constitution and Local Union bylaws. The UAW may notify CalArts of this authorization. This authorization and direction is voluntary on my part and shall remain in effect while I am employed by CalArts unless and until I revoke it by notifying the Union of my decision to revoke. I authorize and direct CalArts to deduct such amounts from my pay and to remit the same to the Union at such times and in such manner as may be agreed upon between CalArts and the Union at any time while this authorization is in effect.

This authorization is made pursuant to the provisions of Section 302(c) of the Labor Management Relations Act of 1947 and otherwise. Contributions or gifts to the UAW are not deductible as charitable contributions for federal income tax purposes.

Voluntary Contributions to the UAW Voluntary Community Action Program (V-CAP)
Through voluntary contributions to the UAW Voluntary Community Action Program (V-CAP), we have a stronger voice in local, state, and national politics! By contributing, we help elect pro-worker and pro-education candidates who can influence state and federal decisions on key issues, like funding for research and higher education. I am a U.S. eligible voter or U.S. Permanent Resident and I hereby join UAW members across the country to have a stronger voice in my community by contributing to the UAW Voluntary Community Action Program (V-CAP). I assign to UAW V-CAP the following sum or percentage of my gross pay each and every month, and voluntarily authorize my Employer each month to deduct this amount from my wages and remit it to UAW V-CAP. I understand that signing this authorization and contributing to UAW V-CAP are not conditions of employment or UAW membership, that contributions are not tax-deductible, and that my status, rights, and benefits in UAW will not be affected by whether or how much I contribute. I also understand that UAW V-CAP will use the money received to make political contributions in connection with local, state, and federal elections as decided on democratically by UAW members.
